The Freedom of Information Act (FOIA) 2000 gives you the right to request access to all types of recorded information held by public authorities, including NHS organisations.

Before making a freedom of information request, please check to see if the information you are looking for is already published by us on this webiste as part of our Publication scheme. We're committed to keeping you informed about your local NHS services in the Surrey Downs area, so we aleady publish a wide range of information about what we do. This includes policies and procedures, governing body papersfinancial information and other publications, such as our annual reports.

We've considered the Information Commissioners Model Publication Scheme in drafting our scheme.

How do I make an FOI request?

Anyone can make a request under the Freedom of Information Act but requests must:

  • be in writing
  • state your real name (we cannot accept requests submitted under a pseudonym)
  • your address (email addresses are acceptable)
  • contain a description of the information you wish to obtain
